PEAK Mastery is working toward opening as a tuition-free public charter school in Cache Valley. While official enrollment is not yet open, we want to hear from families who are seriously considering PEAK Mastery for their children.

Your response will help us better understand anticipated enrollment, grade-level interest, and family demand as we continue planning for the proposed school.

Completing this form tells us that your family is seriously interested in enrolling at PEAK Mastery when official enrollment becomes available.
This form does not enroll your child, reserve a seat, guarantee admission, or provide preference in any future enrollment process. Official enrollment will take place separately and will follow all requirements applicable to Utah public charter schools.
Yes, you can still complete the Intent to Enroll Form. The Interest Survey helps us understand general community interest and family preferences. The Intent to Enroll Form is for families who have moved beyond general interest and are seriously considering PEAK Mastery for their child(ren).
